Duplicates appear in my history as "URL Fetching failed", when set to "Discard".

I'd rather they didn't appear at all.

Are you sure the show as url fetching failed? Because that error should only be thrown if there's some connection error.
Maybe it's a bug though.
Could you enable Debug logging in the Status window and then after it happens again click Show Logging in the Status window and send the log to me at safihre@sabnzbd.org.
