Duplicates being downloaded hours later

I'm using Sickbeard with SAB and SAB will download a file but hours later, sometimes as much as a full day, will redownload the same file. I have duplicate detection on but the filenames are different: Show.mp4 and Show nzb.mp4, I do not have a watched folder or NZB backup folder, and Sickbeard is listing the episodes as "Downloaded" successfully the first time not just "Snatched" or "Wanted", they're being processed and moved successfully both times and are replacing a same-sized file just a few hours later, "Download Propers" is also unchecked. I'm asking here first because I don't know if it's Sickeard or SAB's "fault" and would appreciate some expert input.

Removed your log as it's against the rules.

It looks like a Sickbeard problem, though. Sickbeard is finding what it thinks is a new release and sending it to SABnzbd. It may be a quality thing, where Sickbeard thinks it's upgrading quality but really isn't.
