When i put a .nzb file in the watch folder sometimes it gets double in my download queue.
I can't figure out why this happens from time to time. it also happens when i use a category prefix like {{categoryname}}filename.nzb

Check your connection in Status and Interface settings window.

Do you use the firefox extension nzbdstatus? It adds downloaded files to your queue automatically unless you turn it off. This means it can be added by nzbdstatus and then picked up in the blackhole at the same time.
You can save the file anywhere else and nzbdstatus will add it, or turn off the function in the options, it is labelled "Send nzb files to this daemon".
