If you set a script for a category, then SABnzbd will run the script for any NZB of that category.
When software like Mylar sends an NZB to SABnzbd, it will set a category.
However, it is possible that Mylar tells SABnzbd explicitly not to use the script.
Like when erroneously t sends an empty name for the script.

BTW: you either set the script for each category yourself (Config->Categories) or Mylar does that for you.
I don't how Mylar works.
